Medical Care Visa

Documents Required

All documents listed below are mandatory. Each applicant must submit their own set of documents.

Important: Most documents required for your visa application must be recent and dated within the last 30 days at the time of submission. Ensure all paperwork meets this requirement to avoid delays or rejection.

Application Form

• Completed and signed Schengen visa application form (in English or Swedish)

Photograph

• One recent passport-sized colour photo (not older than 6 months)

Passport

• Valid passport or travel document
• Must be valid for at least 3 months beyond the intended departure from the Schengen Area
• Must contain at least two blank pages
• Copy of the passport’s biodata page

UK Residency

• Provide a UK eVisa printout from UKVI with a valid share code

Travel Medical Insurance

• Valid for all Schengen countries
• Minimum coverage of 30,000 EUR
• Must include emergency medical care, hospitalisation, and repatriation
• Must cover the entire duration of the stay

Proof of Travel

• Confirmed return travel booking to the UK
• If travelling onward, provide proof of entry into the destination country (e.g. visa, onward ticket)

Proof of Accommodation

• Hotel or facility booking confirming accommodation in Sweden
• OR official documentation stating that accommodation is provided by the medical institution
• If accommodation is not provided, proof of financial means to cover costs

Proof of Finance

• UK bank statements for the last 3 months showing the applicant’s name and current balance
• OR credit card statement indicating available limit
• If financially supported by a parent or spouse:
  • Birth certificate or marriage certificate (as applicable)
  • Sponsor’s bank statements (last 3 months)
  • Signed declaration of financial support

Proof of Employment (if applicable)

(Documents must be recent and dated within the last 30 days at the time of submission.)

For employed applicants

• Recent letter from employer confirming position, salary, and company registration details
• Last 3 months’ payslips

For self-employed applicants

• Letter from accountant, solicitor or banker outlining business activity and annual income
• Self-assessment tax return

For unemployed / pensioners / others

• Supporting documentation confirming regular income or ties to the UK

For students

• Recent signed letter from your UK academic institution
• Must include study details, weekly hours, and attendance record

Purpose-Specific Documents: Medical Treatment

• An official letter from a medical institution in Sweden confirming the need for treatment
• The document must outline the type of treatment, duration, and appointment details
• Proof of pre-payment for treatment (if available)
• OR documentation showing sufficient funds to cover the full cost of medical care and associated expenses

Photo Specifications

• Full face, directly facing the camera, with a neutral expression and eyes clearly visible
• Taken against a plain, light-coloured background (preferably white or light grey)
• The image must be sharp, in focus, and not digitally altered
• Head coverings are not permitted, except for religious reasons; in such cases, facial features must be fully visible from the bottom of the chin to the top of the forehead, and from ear to ear
• The photo must not contain any shadows, red-eye, or reflections
• Glasses are permitted if they do not obstruct the eyes or cause glare; tinted or dark glasses are not acceptable unless medically required
• The photo must not be more than 6 months old and must clearly resemble the applicant at the time of submission
• Size should be approximately 35mm x 45mm (standard EU/Schengen size)
• Only required for applicants under the age of 12, or for those who have not submitted biometrics for a Schengen visa in the last 59 months
